Marketplace Insurance
I Had Marketplace Insurance. Where's My Tax Form?
If you or someone in your household had Marketplace health insurance, the tax form you're usually looking for is Form 1095-A. You may be able to download it now instead of waiting for another copy in the mail.
HealthCare.gov
- Sign in to your Marketplace account.
- Select the application for the tax year you're filing—not the current coverage year.
- Select Tax Forms.
- Under Your Form 1095-A, choose Download PDF.
- If you see a form marked Corrected, use the corrected version instead of the earlier one.
Minnesota / MNsure
Sign in to your MNsure account and check Notifications or your Enrollment Dashboard / My Inbox for the 1095-A attachment.
Don't race to file without it. If you had Marketplace coverage, make sure you have an accurate 1095-A before filing. You can receive more than one 1095-A if plans, household members, or coverage changed during the year. Form 1095-A information is used with Form 8962 to reconcile the Premium Tax Credit when required.
If your coverage came through a state Marketplace other than HealthCare.gov or MNsure, use that Marketplace's official account or support site to retrieve the form.
